Medication Administration Authorization Form (1 form for each medication - Example: 1 form for EpiPen and 1 form for Benadryl)
Tuition Payments
For the 2024-2025 school year, we will be using Procare, an online payment portal. This service gives families the option to enroll in automatic payments and access statements and receipts at any time.
All families with a student enrolled will be emailed an invoice from Procare for your monthly tuition. There are nine payments due per year, with the first due in May (or at the time of registration) and the rest due September through April.
The invoice will be sent on the first of the month with a due date of the fifth. Please do NOT use cash, checks, or PayPal, as we no longer accept these payment methods.
Upon receipt of the September invoice, you will need to set up your online profile and add a payment method of your choice. If you choose to use ACH or a debit card, there are no additional fees to the payer. Credit card transactions will have a 2.7% fee + $0.30 added to each transaction, which will be added to your invoice upon checkout.
Due to financial regulations, MHNS must absorb the fees associated with debit cards. We kindly ask, if you do not want to use a credit card, to set up ACH to save costs for MHNS.
